Health & Dental Plan: Dental Opt-Out / Opt-In Deadline

Opt-Out

If you are an eligible student and have comparable dental coverage, and have not opted out in the prior year, you may apply to opt-out of the Dental Plan. Opt-outs must be submitted in your entry semester of the current benefit year. The opt-out form MUST be submitted online, along with providing proof of comparable coverage at: https://www.mystudentplan.ca/uofguelph/en/forms

Please Note: if you opted-out of the Dental Plan last year, your opt-out will carry forward to this benefit year and you will NOT be required to opt-out again.

If you opted-out of the Dental Plan last year and wish to enrol back onto the plan as of September 1, 2021, you may submit an opt-in form and pay the applicable fee at: https://www.mystudentplan.ca/uofguelph/en/forms
